Forum on 11 June spotlights support for people leaving hospital in Hackney

Care and convalescent services for Hackney residents after leaving hospital come under the spotlight at the next NHS Community Voice on Thursday 11 June.
Hackney’s patient forum will look at Reablment and Intermediate Care services following the closure of Median Road Resource Centre.
Median Road was a 37 bed short stay care unit for people with care, convalescent or rehab needs which closed unexpectedly at the end of March 2015.
Residents, patients and carers have raised concerns about services for local people leaving hospital now the unit has closed.
Come along and find about rehab and interim care services, how closure of Median Road affects patients and carers and what services are planned over the long term.
